Request for Evaluations / Special Education Services

Please fill out the following forms:

  1. BCPS Registration (this is for evaluation purposes only)

  2. BCPS Release Form (1)- Write Teach Florida in the space at the top.

  3. BCPS Release Form (2)- Write The Jewish Academy in the space at the top

  4. Teach Florida Release

  5. BCPS Request for Special Education Services (Check Educational Evaluation and Private School Services at the top)

Send all documents, along with a copy of your Driver's License and child's Birth Certificate to:

Mrs. Walker will then send the request to the BCPS Private School Services Office and cc you.
